Find Provider NPI or PECOS name or Facility NPI

You can use our NPI Lookup feature to quickly view a Provider's or Organization's information as stored in the NPPES NPI Registry database and the PECOS Ordering/ Referring File.

We automatically search for Alternate Provider Name (AKA) and Organization Name (DBA) names for your convenience.

National Provider ID: Enter all or a portion of the NPI and click the Search button. 
Organization Name: To search by Organization Name, enter part or all of the Organization Name.

Use '%' as a leading wildcard character -- A search for '%hospital' will bring up every organization with 'hospital' in the orgranization name or DBA name.

You may also select the State where the Organization's practice is located with the 'Choose a State' box if you so desire.
 
Provider Last Name: Enter the first portion or all of the Provider's Last Name.

You may also include the first portion or all of the Provider's First Name in the 'Provider First Name' box.

Use '%' as a wildcard character. A search for '%son' will bring up every provider with 'son' in the name or AKA, such as Smithson or Johnson.

You may also select the State where the Provider's practice is located with the 'Choose a State' box if you so desire.
 
Provider First Name: Enter the first portion or all of the Provider's First Name only if you are searching by the Provider's Last Name / First Name.

You may use '%' as a leading wildcard character; '%illi' will bring up all first names that include 'illi', such as Phillip or William.
 
State: If you wish to narrow down your Organization or Last Name search, you may select the State where the Provider or Organization practices. 
Phone or Fax Number: Enter at least 7 digits of the Entity's phone or fax number with no punctuation.

Legacy ID: Enter an alternative ID such as a UPIN, Medicaid or Medicare PIN
This must match exactly, so do enter hyphens and other punctuation as needed.
